HOW WE HELP
Many nonprofits want to offer health services—but Medi-Cal rules and paperwork can be tough to figure. That’s where we come in.
- Handling contracts and paperwork

TRAINING & CAPACITY BUILDING
The CBO Medi-Cal Capacity Building Series (Medi-Cal Track) is a collaborative partnership with and Health Plan of San Joaquin. It was founded to support Medi-Cal capacity building and training for community-based providers.

WHY IT MATTERS

By making it easier for nonprofits to offer Medi-Cal services, we can:

- Address mental health and wellness needs
- Build a stronger, more connected network of care
OUR CARE COORDINATION NETWORK
The AFW aims to improve health outcomes by coordinating care and addressing social determinants of health through community health workers (CHWs). Selected care coordination agencies will employ CHWs trained to navigate and close Pathways addressing health-related social needs (HRSN), most notably:
- Access to care and benefits navigation
- Employment and transportation support
- Housing through navigation, tenancy, and sustaining services support
- Medication screening, reconciliation, and adherence
- Asthma remediation through qualified supplies and physical modifications
- Food access and security, especially Medically Tailored Meals for those with chronic conditions
